520 _aAbstract: Background: Caesarean section rates have increased worldwide in recent decades, Caesarean section is an essential maternal healthcare services, however, it has both maternal and neonatal adverse outcomes, therefore this study is aimed to estimate the pooled prevalence of Caesarean section deliveries and to determine the indications and fetal-maternal outcomes of Caesarean section deliveries at Geita regional referral hospital. Objective: To determine the prevalence, indications and fetal-maternal outcome of caesarean sectionn among adult pregnant women delivering at Geita regional referral hospital. Methodology: A prospective descriptive Hospital -based cross section study was done for determining prevalence, indications and Fetal-maternal outcome of caesarean section among adult pregnant women delivering at the obstetrics unit of Geita regional referral hospital from January to march 2023 ,Simple random technique was applied to the sample size of 219 people ,the data were obtained by the use of structured questionnaire, the collected data were analyzed using SPSS version 20 software. Results: In three month time (January 2023_March 2023), there were 1,285 deliveries, of which 272 were delivered by cesarean section which gives the overall rate of 21.2%.The most common indications for cesarean section were Cephalo-pelvic Disproportion 71(32.4%) followed by obstructed labor (32 %) ,the most maternal complication was surgical site infection 42 (19.2%) and most neonatal complications were neonatal sepsis 14 (6.4%) followed Low Apgar score 13 (5.9%)and births asphyxia 12 (5.5%). Conclusion: In this study, there is a steady increase in the rates of cesarean delivery in five years' time, the overall rate of C-section in Jugal Hospital was 29.7%. This rate is higher than the World Health Organization standard. Policies and efforts aimed at decreasing the unnecessary cesarean sections should be promoted and implemented at each health facility.
