520 _aAbstract Background: Borrelia spp. Infection was known to cause recurrent fever in many parts of the world particularly those with high density of vectors. It is one of the vector borne zoonotic neglected tropical disease which frequently misdiagnosed as malaria or typhoid fever. Despite having a global distribution, little is known about its epidemiology in Tanzania Objective: the aim of the study was to determine the prevalence and associated factors of Borrelia spp infection among adult febrile patients attending outpatient clinics in Mwanza, Tanzania. Methods: The study was across sectional hospital based study involving 455 adult patient with fever attended outpatient clinics in different hospitals in Mwanza, during the study period (June-August 2018).sociodemographic data and the relevant information was collected using structured data collection tool. Detection of Borrelia spp. Infection in blood was performed by using Giemsa stain according to standard operating procedure (SOP). Data was analyzed by using STATA version 13 software according to the objective of the study. Results: the total of 455 adult febrile of 72.9= 1.8 C. Majority of the patients 389(78.9%) were from urban areas. Among 455 febrile patients enrolled, none of them was found to be infected with Borrelia spp Conclusion and recommendation: Borrelia spp. Is not among the cause of fever among febrile patients in Mwanza. With decrease trend in malaria and typhoid fever, there is paramount need to consider other causes of febrile illness in Mwanza
