Background: Acute lymphoblastic leukemia is the type of cancer in which the bone marrow makes too many immature lymphocytes, it is the most common cancer in children with the peak incidence between 2 to 4 years of age. Due to current advanced to treatment has greatly improve the outcomes of childhood All. However, there is little information concern childhood ALL in developing countries.

Methodology: This is a retrospective cross-sectional hospital-based study, conducted at oncology department Bugando Medical Centre, Mwanza. From 2010 to 2018, these included 86 patients’ files aged 1 to 15 years that were selected by consecutive sampling. Data were filled in excel 2016 spreadsheet and analysed by STATA version 15.

Results and Discussion: 8(9%) patient were alive, 41(48%) of patient died during treatment and 37(43%) of patient has unknown outcomes status. With the mean average age of patients’ participants was 7, and male were more prevalent 71% than female with 29%.

Conclusion and Recommendation: Leukaemia survival remain below in developing countries, but the survival rate for ALL has significant improved due to standardization of regimens and better staff. Improvement of outcomes will require to improve awareness, early access to treatment, improved supportive care and improved medical too,
