Introduction: Hypertension is a persistently abnormal elevation of blood pressure noted above 160/95 mmHg or above 140/90 mmHg based on three readings on separate occasions is considered to be hypertensive [1-2]. Hypertension can be primary (essential) which is idiopathic and constitute over 90% of the patients or secondary hypertension (10%) due to a pre-existing disease such as renal disease and endocrine disease such as Cushing syndrome. 

Problem statement: Hypertension particularly primary HTN may be asymptomatic and may also be accidentally diagnosed. Tanzanian’s people do not have a tendency of regular medical checkup and accidentally diagnosed to the hypertension. 

Objective: To determine the prevalence, distribution and factors associated with hypertension among undiagnosed adults in Magu district. 

Methodology: This was a cross sectional descriptive study which conducted at Magu district in Mwanza. Participants were be recruited consecutively until 300 estimated sample size was achieved. The data obtained was entered and then analyzed using the statistical package for social science (SPSS) version 20.

Results: A total of 300 individuals were attended from different wards in Magu district during the study, where 193 were female and 107 were male, 18 individuals were known hypertensive (blood pressure140/90 mmHg) although were asymptomatic, therefore the prevalence of undiagnosed was 22.0%
