TY - BOOK AU - Erasto Phares Wambura AU - .John Igenge AU - Flavian Simula TI - Short-Term Outcomes of Tubularized Incised Plate Urethroplasty and Associated Factors among Children with Hypospadias at Bugando Medical Centre in Mwanza Tanzania PY - 2023/// CY - Mwanza, Tanzania PB - Catholic University of Health and Allied Sciences [CUHAS – Bugando] : KW - N2 - Abstract: Background: Hypospadias is an abnormal ventral opening of the urethral meatus with a prevalence of 3 for every 1000 news born and is characterized by distal 50%, mid 30%, and proximal 20%, but In Bugando medical Centre, Tubularized incised plate urethroplasty is a preferred technique of hypospadias repair techniques due to its simplicity and have good cosmetics results with low complications rates compared to other techniques of hypospadias repairs, despite that in unpublished data the rate of complications developed in patient after Tubularized incised plate urethroplasty in among 10 patient operated, about 3 patients had developed complications as the short-term outcome, there are factors that are associated with the development of urethral complications in hypospadias children like, age of the patient, hypospadias site, suture size, and surgeon experiences. But these factors are not known in our hospital setting. Objective: To determine factors associated with short-term outcome of Tubularized incised plate urethroplasty in hypospadias children here at Bugando Medical Centre. Methodology: This was a crossectional study with prospective components, which conducted at Bugando medical Centre Mwanza Tanzania in Urology department from January 2023 to June 2023.Total of 90 children with hypospadias with age ranging from 4 months up to 15 years of age children with hypospadias and who had TIPU and followed for 30 days was included in the study after screened to inclusion criteria, children with proximal hypospadias who had secondary surgery repair and other techniques apart from TIPU was excluded from the study. A standard structured questionnaire and checklist collecting tool incorporated unto it was used to collect data as per objectives, and all data were first entered into an Excel sheet, cleaned and then transferred to STATA version 15.0 software for analysis according to the objectives of the study. Bivariate analysis was done and fisher-exact score used to find associations between the short-term outcomes of TIPU with factors associated and a P-value of <0.05 was considered significant. Results: 90 patients with age range 4 month up to 15 years old, has a median age of 2.25 years, the age of patient was grouped into two group <2years and 2> years of age. The patient was operated with different urologist by using TIPU as the only surgical techniques and followed up for 1 month post operative, the frequency of distributions was glandular in 40 (44.44%), coronal in 29 (32.22%), subcoronal in 15(16.67%), distal penile in 4(4.44%), and midshaft in 2(2.22%), the complication rate was 34.44% equal to 31 who developed complications as short-term outcomes and 65.56% equal to 59 had heals without complications. Age of the patient at primary surgery, and surgeons experience were not associated with development of complications. The hypospadias site has significant associated with development of urethral complication with p value of 0.029, Conclusion.: Despite to high success rate of TIPU but complication rate is still high ranging from mild like hematoma and fever which can be managed with conservatives and other are serious such has dehiscence and which can be managed with consecutive surgeries. whoever the age of patient for primary hypospadias repair is not an independent risk factor for development of complications after hypospadias repair and hence the TIPU can be performed at any age in children, there is association between the hypospadias type on meatal location with development of complications with scientific significant (p value=0.029), suture size, surgeon experience also was not associated with development of urethral complications ER -
