Factors Associated With Late Diagnosis and Delayed Treatment of Congenital Heart Disease among Children Attending Pediatric Clinic at Bugando Medical Center Mwanza, Tanzania.

Therefore it is crucial to determine factors related to delayed diagnosis and treatment of CHD. Methods: A cross-sectional study was conducted to assess the associated factors for late diagnosis and delayed treatment among children with CHD attending pediatric clinic at BMC starting from February 2023 to July 2023. Mothers or legal guardian of Children with CHD attending pediatric clinic at BMC aged 0 to 12 years were interviewed using questionnaires and all children with CHD confirmed by Echocardiography aged 0 to 12 years were included. Results: a total of 217 children were included with a median age of 2 months, female predominance of 51.6%. The proportions were 74.5% and 62.2% for delayed diagnosis and delayed treatment respectively. Lack of financial resources was the leading cause of delayed diagnosis about 15.4% followed by lack of awareness 14.8%, lack of insurance and need of referral to higher facility to make the diagnosis. For delayed treatment, lack of financial resources 17.6% followed by lack of insurance 16.5%, and referral hospital being too far were the commonest causes. Conclusion: 7 out of 10 children with CHD the diagnosis is delayed and 6 out of 10 the definite treatment is delayed. The leading cause for both delayed diagnosis and delayed treatment was lack of financial resources.   Recommendations: Health professionals at different levels should have adequate knowledge about screening of children during birth especially those who are at risk mothers. But also health professional should give education about CHD to the mothers about the common symptoms and signs of CHD although are nonspecific and encourage them to seek medical assistance. Policy makers should also consider starting of CHD awareness raising campaigns in the community and find a way to assist those in need of cardiac operation and they don’t have adequate financial resources. I am recommending to researchers to use this study as a starting point to do more studies about congenital heart diseases as for current situation this study is the only one has been done in our setting.
