Indication and Complications of Colostomy among Patients attending Surgical Department at Bugando Medical Centre.

Indications may vary from region to region. Attending morbidity and mortality is significant. The objective of this study was to determine indications and complications of colostomies of patients attending surgical department at Bugando medical centre. Methods: A cross sectional hospital based study involved 92 patients with colostomies was conducted at Bugando medical centre from September 2019 to February 2020. Socio-demographic data and other relevant information was collected using structured questionnaire. Data was analyzed using SPSS version 2.0 software as per study objectives. Results: Of the 90 cases enrolled, 52 (57.8%) were males and 38 (42.2%) were females. The ages ranged from 3 weeks to 28 years with a mean age of 2.17 years. The most common indications of colostomy were imperforate anus, 39 (43.3%), rectovaginal fistula, 24 (26.7%), Hirschprung disease, 12 (13.3%), abdominal injuries, 5 (5.6%), and sigmoid volvulus, 3 (3.3%). Other indications like ectopic anus, sacrococygeal teratoma, anorectal trauma, cloaca, rectogluteal fistula and bucket handle fistula type were encountered in 7 (7.8%). The commonest type of colostomy done was double barred stoma, 72 (78.1%), Hartmann’s colostomy, 11 (12.1%), loop colostomy, 4 (4.4%) and spectacle, 3 (3.3%). Overall, complications were seen on 17 (17.6%). The common complications were prolapsed stoma, 8 (8.8%), skin excoriation, 5 (5.5%), retraction of stoma, 2 (2.2%) and surgical site infection, 1 (1.1%). Conclusion: Anorectal malformations (imperforate anus and rectovaginal fistula) and Hirschprung disease are leading indications for colostomy in our setting especially pediatrics patients. Abdominal injuries and other surgical conditions accounts for most common indications beyond pediatric groups. A construction of colostomy in patients carriers high frequency of complications, mostly related to caring of stomas.
