Prescribing Patterns of Drugs for Type 2 Diabetes Mellitus During Patient First Visit Compared to the National Treatment Guidelines at Bugando Medical Centre (BMC)

Abstract:
Background: Several new oral anti-diabetic drugs have been launched in the last few years. The Tanzania STG recommends metformin as the first line initial therapy in diagnosed T2DM patients, and sulfonylureas as the second line initial therapy if metformin is contraindicated. The aim of this study was to determine patterns of medication initiation in T2DM as compared to the guidelines.
Methodology: A hospital retrospective cross-sectional study was conducted at Bugando Medical Centre, Mwanza, Tanzania. Random sampling was done to obtain a sample of 360 T2DM patients who attended Bugando Medical Centre outpatient clinic from January to December 2018. Data was collected through patient file review then it was entered and analysed using SPSS 20.
Results and Discussion: The mean age of the patients was 59.79 years. Females were 196 (54.4%) and males were 164 (45.6%). Metformin was mostly used during therapy initiation in T2DM patients (37.8%0 followed by insulin (20.3%) and glibenclamide (16.4%). Metformin or a sulfonylurea was prescribed in 73.89% of T2DM patients during therapy initiation.
Conclusion and Recommendations: Metformin was the most frequently used drugs during therapy initiation, followed by insulin and glibenclamide. Adherence to STG and other international treatment guidelines during the initiation of therapy in T2DM is low compared to other developed countries. Further studies should be done to determine the factors affecting drug selection by the prescribers during treatment initiation in T2DM patients.
