Abstract:
Irrational prescription caused serious problem in health sector, its common to find inappropriate prescription from physician brought by patient to community pharmacies. Study aim to access pharmacist and to assess common problems in prescription occurs in community pharmacist and measure they take in solving problems.
Methodology: 48 number of participants were included. Study was cross sectional involving questionnaire it include registered pharmacist in Mwanza city, and analysis was done by using STATA ethical consideration was done and permission letter was obtained from school of pharmacy – CUHAS, sample size used was small and error was likely to occur.
Results: 48 number of participants were interviewed and different measures such as changing prescription 45.83% and returning prescription 10.42%. Common problems identified were polypharmacy 8.33%, incorrect dosage 12.50%, incomplete information of patient were 4.17%, drugs prescribed in brand name 14.58%
Conclusion: Prescribing should not be considered as individual activity, communication between specialist, other prescriber and pharmacist is essential for the most rational prescribing of medicine, the process of planning to reduce inappropriate prescribing should involve all the stakeholder of health care system and contact information of prescribers should be written in prescription form.
