Prevalence of Ureteric Injury, Associated Factors and its Short Term Surgical Management Outcome Among Women with Urinary Incontinence Attending Bugando Medical Centre.

Abstract:
Background: Iatrogenic ureteral injuries can occur during various obstetrical & gynaecological, abdominal pelvic and retroperitoneal surgical procedures, urological, colorectal and vascular operations.
Objective: The aim of our study was to determine the prevalence, associated factors and short-term surgical management outcomes of Ureteric injury, among women with urinary incontinence attending Bugando Medical Centre.
Methods: A retrospective analysis of clinical data (medical records and operative reports) patients with urinary incontinence over period of 5 years from 2012 to 2016, we were able to analyze 500 files that fulfilled the criteria for the study.
Results: We went through the hospital database in the period of 5 years, The total number of collected files was 527, of these, 500 files were taken for analysis, and 27 files were dropped due to insuffient data. The mean age of the study population was 28 (±9.8) years. The prevalence of ureteric injuries was 7.2% (36/500). Caesarian section, (p-value 0.001) and obstructed labour (p value 0.009) had true association with ureteric injury. All injured ureters had Ureteneocystostomy done as corrective surgery. There were no complications in 33 (91.7%) patients’ 2 (5.6%) had sepsis and 1 (2.8%) had broken anastomosis.
Conclusion: The prevalence of ureteric injury during obstetrical/gynecological surgeries is high. Obstructed labor and caesarean section were significant factors associated with ureteric injury during surgery. Management outcomes for these patients treated at Bugando Medical Centre was excellent.
