Summary, etc. Abstract:<br/><br/>Background: Gastrointestinal leakage still remained to be the most feared and catastrophic general surgical condition worldwide. It is associated with a number of complications from local infection, sepsis and multiorgan failure. These complications in turn usually lead to prolonged hospital stay, increased cost of treatment and even death. There is limited data regarding early postoperative outcomes and their predictors in patients with Gastrointestinal leak. This study aimed to determine the clinical profile and factors associated with early postoperative outcomes among patients with GI leak at Bugando Medical Centre (BMC) and Sekou-Toure Regional Referral Hospital (SRRH) Mwanza, Northwestern part of Tanzania<br/><br/>Methods: This was a longitudinal study involving all patients presenting at BMC and Sekou Toure Regional Hospital with Gastrointestinal leak over a period of five months between February 2024 and July 2024. The study included all patients presenting with Gastrointestinal leak in the general surgical ward at Bugando Medical Centre and Sekou Toure Regional Referral Hospital (SRRH) and who consented for the study. Data collected using a pretested, structured questionnaire and analysed using STATA version 15. Before the commencement of the study, research proposal approval from the department, Ethical clearance was requested from the Joint CUHAS/BMC Research, Ethics and Review Committee.<br/><br/>Results: On univariate logistic regression age of the study participants when 30 days in hospital mortality compared those who were 50 years and more had 19.5 times more likely to succumb in hospital mortality compared to those below 30 years of age (OR = 19.5 95%CI 7.86,48.1). Among other factors high level of ASA classification (p-value 0.009), hypoalbuminemia (p value .047) and HIV prevalence (p value <0.01) were all found to be significantly associated with 30 days in hospital mortality. Among the intraoperative and postoperative factors only part of the GI involved (p=0.009), duration of surgery of more than 3 hours (p=0.026) and intraoperative contamination (p=0.001)were found to be significantly associated with 30 days hospital mortality, other factors which were found to be significantly associated with post-operative complications included drainage placement intraoperative (OR 6.6 95%CI (2.4 -18.1) p=0.038 ), NGT insertion post-surgery (OR 2.25 95%CI (0.027-4.94)) and intraoperative contamination/ spillage of GI content intra peritoneal during surgery (p=0.013).<br/><br/>Conclusion: In this analytical cross section study that involved 118 study participants admitted at BMC and SRRH. This study has provided in depth patients characteristics which affect clinical outcome of patient with GI leak. We found abnormal albumin level, electrolyte derangement and anemia are common among patients with GI leak. There is association between low albumin, HIV status, duration of surgery, ASA classification and poor early postoperative outcome.<br/><br/>
