Summary, etc. | Introduction

1.1: Background Information

Worldwide children whose age is less than 5 years’ experience on average 2 to 3 episodes of diarrhea every year and consequently 1.87 million children will die from dehydration associated with diarrhea disease[1] Despite the reported 68% global reduction in number of deaths because of diarrhea in 2003 and 2009),[2] this reduction may be due to correct case management as per Standard treatment guideline recommended by WHO and use of Oral rehydration therapy as the key stone in management [3].Diarrhea has remained among the common cause of morbidity and mortality in children in developing countries. [4]

In Sub Saharan Africa and South Asia account for 90% of globally diarrhea deaths in children and In Africa the burden of diarrhea remains high for an estimated 30 million cases of severe diarrhea and 330,000 deaths in 2015[5] the severity of diarrhea is aggravated by lack of access to sufficient clean water, poor sanitation, hygiene practices, contaminated weaning food and lack access to and lack access to adequate and affordable health care.

In Tanzania a study conducted at Mkuranga district reported that diarrhea was the fourth contributor of outpatient visit and the fifth cause of mortality among under five children in the year of 2009. This revealed that diarrhea disease have been and continue to be a major child health challenge and this is attributed to the poor living conditions and social status as well as inadequate knowledge and practices of mothers of under five children on how to proper care of children, lack of provision and access to safe clean water, inadequate sanitation and lack of hygiene maintenance at home. [6]

Another study about diarrhea in children under five years from Ifakara Tanzania mention that most mothers had negative attitude towards the prevention of diarrhea in under five children, this implies that mothers are not aware on the use of ORS for management of diarrhea at home, giving ORS to the children who are experiencing diarrhea is important because can prevent dehydration which is the most complication of diarrhea in children. [7]

The World Health Organization recommends the use of oral rehydration salt(ORS) and also zinc supplementation in diarrhea[8] .Oral rehydration salt solution is a lifesaving treatment that is safe for people to use in their homes. The use of oral rehydration therapy is the first choice in diarrheal disease control efforts. The fluids given could be either ORS or recommended home based fluids example soups, rice water, yoghurt drinks or clean water. It was advised that all children with diarrhea should be given more fluids to drink to compasate the loss of fluids and feeding should not be stopped during diarrhea [9].

Death due to diarrhea are mainly due to dehydration which can be prevented by prompt treatment with home based fluids as sometimes attending at health care facility for further management. Hence management of diarrhea should starts with home available fluids, thus educating population regarding home based management of diarrhea is the effective prevention practice of diarrhea [10]

Therefore, Diarrhea disease among under five children remains a serious challenge as many under five children are admitted in the medical wards at Bugando medical center.

So this study aim to assess mothers knowledge, attitudes and practices prevention towards diarrhea among under five children at Bugando medical center in Mwanza Tanzania.

1.2 Problem Statement

In Tanzania confirm that diarrhea in a children of under five years old remains a public health problem in the country the national prevalence of childhood diarrhea is 12.1%[11] is somehow lower than the previous survey conducted in 2010 and revealed that the prevalence of childhood diarrhea was 15%. Under five children with poor nutrition status as well as those exposed to unsafe drinking water are more susceptible to severe diarrhea. However the severity of diarrhea prevented by safe drinking water, improved sanitation, washing hands with soap and at home by giving children Oral rehydration solution while you are prepared going hospital for further management.

In Mwanza a study conducted on prevalence of diarrhea among under five children ([12] and reported that the prevalence of 11% with childhood diarrhea in Mwanza city however risk factors associated with childhood diarrhea are unplanned settlement and behavior of the mothers and child washing hands during critical time. Household interventions especially household water treatment and safe storage can make immediate contribution to safety component of this target and would significantly contribute to MDG.
