Summary, etc. | Abstract:

Background: TB is among the major public health problem in the world but mostly in developing countries. According to the most recent global report, TB is considered the ninth cause of death worldwide and the leading cause of mortality and it affects mostly immunocompromised people. The epidemiology of TB varies substantially around the World. The highest rates (100/100,000) are observed in Sub-Saharan Africa, India, China, and the Islands of Southeast Asia and Micronesia. In Tanzania, the study was done among people who use drugs attending Medication-assisted Treatment (MAT) Clinics in Dar es Salaam showed the prevalence of TB disease and TB infection was 2.6% and 54% respectively. Detection of MTB is essential for its diagnosis. In our setting the suggested method for MTB detection on tissue biopsy is conventional ZN stain since it has a high specification for MTB Complex. However, this method has low sensitivity which hinders its preference and gives preference to the usage of other staining methods like Auramine/Rhodamine stain which are more sensitive but not specific for MTB complex therefore there is a need to improve ZN Stain sensitivity.

Objective: comparing conventional ZN stain and Microwave heated ZN stain in the detection of AFB on tissue biopsy through determining sensitivity, specificity, and positive and negative predictive value of each method.

Methodology: The study was a retrospective study conducted from June 2022 to August 2022 at Central Pathology Laboratory in the histopathology Unit BMC.It involved retrieval of 136 tissue blocks with Auramine/Rhodamine results archived in the histopathology laboratory at BMC registered from January 2020 to June 2022. Data collected were entered into Microsoft excel for data cleaning and then transferred to a statistical package for social science software(SPSS)version 20 for analysis. The results were presented using tables, bar graphs, and pictures.

Results: Sensitivity, specificity, positive and negative predictive value of conventional ZN Stain and Microwave heated ZN stain were 26.6%,100%,100%,60.5% and 43.8%,98.6%,96.6%,66.4% respectively.

Conclusion and Recommendation: Although in this specific study both conventional and microwave heated had low sensitivity the latter had relatively high sensitivity compared to the former, this advantage needs to be embraced and further studies preferably prospective design should be done to explore more
|
