Summary, etc. Abstract:<br/><br/>Introduction: Multidrug resistance negative bacteria (MDR-GNB) of the family Enterobacteriaceae is the currently one of the group most challenging both in developed and developing countries. Intestinal carriage is the main reservoir of these organisms. The Gastro intestinal tract (GIT) colonization is associated with a high risk for developing self and cross infections due to MDR-NGB producers, especially in Urinary tract infection (UTI) patients. <br/><br/>Methods: A cross sectional based study involving 799 symptomatic UTI patients in Mwanza city was conducted between April and July 2019. Stool specimens were collected and processed to detect MDR-GNB producing Enterobacteriaceae interpreted according to standard operating procedures. Data were analyzed using STATA version 13. <br/><br/>Results: A total of 799 stool samples from clinically diagnosed UTI patients, 674 (84.4%) were female. The median age was 29 years (IQR) 23-38). Out of 799, 778 (97.4%) were out patients. 54.1% (432/799); isolates were resistant to third generation cephalosporin’s with all of them exhibiting extended spectrum beta lactamase (ESBL) phenotype. Rates of resistance to gentamicin and ciprofloxacin were 73/167 (43.7%), and 95/167 (55.1%) respectively. Being inpatients, hospital admission were independent factors found to predict MDR-NGB carriage. <br/><br/>Conclusion: Colonization of multidrug-resistant enteric pathogens in Mwanza city hospitals has serious impacts on health outcome and mortality. Therefore, requiring the implementation of surveillance, antibiotic prescribing policy, and associated factors that lead to persistence spread of multi drug resistance bacterial in Mwanza city. <br/>
