Summary, etc. Abstract: <br/><br/>Introduction: Psychoactive substance use continues to be major risk behaviours among youths, accompanied with physical and/or mental health complications/consequences and it is becoming increasingly problematic in Tanzania. This study was conducted to examine the reasons and extent of substance abuse among secondary schools students, identify commonly abused substance/drugs among secondary school students and identify student’s knowledge on the effects of substance abuse. <br/><br/>Methods: A descriptive cross sectional survey using questionnaire to collect information on the prevalence of substance abuse among secondary students in Magu district was done at Bujora, Bujashi and Kitumba secondary schools involving a total 200 students from form 1 to form IV secondary school students in the age group 11-25 years. Data obtained, analyses was done using SPSS version 17. <br/><br/>Results: A total of 200 secondary school students participated in the study, majority being female 53.5% (n=107). For the last twelve months prior to the study 25.5% (n=51) of students had used the psychoactive substances at least once. Additionally, psychoactive substance use in a lifetime indicated that 15.5% (n=31) were still using psychoactive substance. The proportion of male students who used the psychoactive substance was higher than that of female student 21.5% (n=20) versus 10.3% (n=11). Majority 90% (n=180) had adequate knowledge on types and 87.5% (n=75) were knowledgeable on effects of substance. The commonly abused substance were alcohol 62% (n=124) and cannabis 48% (n=96). The commonly abused substance abuse were relaxation 36% (n=72) followed by experimentation 22% (n=44) and the least reason being easy availability of substance 6.5%. <br/><br/>Conclusion: The study shows that the prevalence of substance abuse among secondary students is slightly low as in the last twelve months prior to study was 25.5% (n=51 and currently was found to be 15.5% (n=31). 90% had adequate knowledge on different types of psychoactive substances and 87.5% (n=175) were knowledgeable on the effects of substance abuse. Much knowledge concerning substance abuse should be given to the youths so that in the long run they will get to understand the effects of substance abuse in life and those who abuse psychological support and social rehabilitation is to be imported. <br/>
