Abstract:

Background: Documentation is the central to the provision of patient-centered care. Documentation principles are applied to detect, prevent and manage drug related problems Documentation systems implementation in Sub-Saharan Africa countries is much scarcer and many countries have been adopting healthcare information systems. Pharmacists must record prescribed drugs, and keep pharmacy record up to date for documentation and information. Registers are to be kept at the pharmacy. IT-supported systems for documentation of pharmacy actions and maintaining patient, financial and legal profiles.

Methodology: A cross sectional participatory observational study conducted in community pharmacies in Mwanza city. A study involved 125 pharmaceutical personnel in community pharmacies, who were asked questions to respond and available documentation tools to be observed. Data were entered and analysed using SPSS version 20.

Results and Discussion: The study involved 103 (82.4%) pharmaceutical technicians, 13 (10.4%) pharmaceutical assistants, 6 (4.8%) pharmacist and 3 (2.4%) nurses. The study revealed 84.4% had excellent knowledge on documentation in community pharmacies. 57.6% of the pharmaceutical personnel have positive attitude towards documentation. The study showed a good practice in recording expired drugs, general sales book register, recording control, prescription only medicine, inspection book, visitors’ book, cold chain register and temperature card record while recording of adverse reactions and complaints was poor.

Conclusion: Despite of excellent knowledge, positive attitude and good practice on documentation, it is important that this positive outlook be maintained by taking into consideration users’ needs such as completely recording all data on electronic systems without paper based recording, training and implementation of best documentation system updates. Mwanza city council, should promote usage of electronic medical systems in community pharmacy settings. The ministry of health should provide community personnel with good and efficient training on use of available systems and low practiced documents such as adverse reactions and complaints.
