Abstract:
Background: tonsillitis is an inflammation of tonsil due to the either viruses or bacteria and the major treatment of choice has remained to be tonsillectomy. The bacteria causing tonsillitis are either endogenous or exogenous. Failure of antibiotic treatment may lead to obstructive sleep apnea, recurrent tonsillitis or pain on swallowing. This study was aiming at determining bacteria pattern on tonsillar surface pre tonsillectomy and tonsillar core tissue post tonsillectomy, among patients scheduled for tonsillectomy at BMC, Mwanza Tanzania.
Objectives: the study has compared to bacteria found on the surface of the tonsils pre tonsillectomy with those found in the tonsil core post tonsillectomy.
Methods: A cross sectional study involving 120 patients who have undergone tonsillectomy was conducted BMC from April to July 2019. Socio- demographic data and other relevant information were collected by pretested structured data collection tool. Surface tonsillar sample pre tonsillectomy and deep tonsillar samples post tonsillectomy were collected from all patients. Culture was performed following the microbiology laboratory standard operating procedure. Data analysis was done through the use of STATA version 13 as per study objectives.
Results: Among 120 patients scheduled for tonsillectomy their samples were examined for bacteria on surface and core and the specimens were sent to histopathology laboratory to examine tissue changes. Normal flora were more prevalent on tonsillar surface while no bacteria growth was more found in core. Pathogens were isolated majority of them were gram positive bacteria a lead taken by staphylococcus aureus followed by GABHS. Many patients whose sample went for histopathology were found with tonsillitis.
Conclusion and recommendation: tonsillar surface is more colonized with bacteria than tonsillar core. Streptococcus pyogenes is more prevalent on surface while staphylococcus aureus is more prevalent on tonsillar core. Culture can be used for screening purposes because has high specificity compared to histology on other side histology can used as gold standard due to high specificity compared to histology. Broad spectrum antibiotics should be employed in treatment of tonsillitis also the dosage amount should be revised in order to combat surface and core bacteria
