A 73-year-old man presented with new left monocular blindness. During the preceding month he had been diagnosed with HIV infection and esophageal candidiasis. The patient's visual problems began 2 weeks before admission and progressed from left, lower visual field loss to total monocular blindness in 3 days.
The patient denied eye pain, headache, nuchal rigidity, right-sided visual loss, and fever but reported fatigue, decreased concentration, unsteady gait, anorexia, and weight loss of 4 months’ duration. Physical examination revealed a left Marcus Gunn pupil. Fundoscopy revealed blurring of the disc margin on the left. The patient was oriented to person but not place or time, and exhibited delayed response to commands. The neurological examination was otherwise intact with no cranial nerve deficits.
